What does the evaluation consist of? DUI Risk Assessment requires: Review of Driving History, Chemical Test Results (BAC), and Objective Tests for Alcohol and Drug use.

Why do I need an evaluation? Most states require persons arrested for driving under the influence of alcohol and/or other drugs (DUI) to undergo an alcohol and drug evaluation before a driving privileges can be granted by the Court.*

What is the cost of an online evaluation? The cost of our online evaluations are $150.00. Call the office to pay for your evaluation, toll free 1-877-368-9909 and get your User ID and Password for your online assessment. Your written report will be available the same business day or the following business day between 9:00 AM and 5:00 PM Mountain Standard Time by email, fax or mail.
